Patient Connection
We are excited to announce the retirement of our patient portal in favor of Patient Connection, a more streamlined tool that integrates seamlessly with our mobile app. Now you have one way to sign in to both our browser-based platform and the Mobile App!

Patient Connection Sign In
Creating an Account
Existing patients can register for an account , create a password, and reset a forgotten password using their mobile number, which is also their user name. Note: a phone number can only be registered once and cannot be used to register for multiple patient accounts.
New patients can access Patient Connection securely using a passwordless login method once we have received an order from their referring provider. Patients will receive an automated phone call and/or text message letting them know it is time to schedule an appointment and they may do so using Patient Connection, the Mobile App, or by calling our scheduling team.
Whether you’re a new or existing patient, you will input your name, date of birth, mobile number, and email address.
